Heritage Court Assisted Living is much more than a Medicaid certified retirement suite. You get all of the benefits of a small community with the comfort and security of a skilled nursing and rehab center right next door. Fountain West Health Center is a nursing care center dedicated to serving residents 65 and older. Located in a picturesque residential neighborhood on approximately a seven-acre tract, Fountain West and Heritage Court provide an array of care services ranging from assisted living to a full range of nursing services in our three nursing care areas. A personal care assistant is available to provide support services with bathing, dressing, or just being there when you need someone. Caring staff is always on duty and can assist you with your medications and other medical needs. Caring and helpful staff provide full housekeeping and laundry services. line¶ Skilled Nursing—“It’s time to go home!” We know the joy that those words bring to our patients, and we have built our entire program around helping you achieve your goals as quickly as possible. Our therapy services are offered six days per week by onsite therapists. Your therapy is delivered by dedicated professionals committed and focused on your goals. Our staff assists you throughout your journey from start to finish. We specialize in a variety of services ranging from orthopedic recovery, IV therapy, wound care, to general strengthening, all to help get you home faster.

A top priority of our program at Fountain West is to offer a variety of activities, ensuring that people are always engaged. Our activities include ladies’ luncheon, men’s luncheon, validation therapy, weekly religious services, exercise group, Wii bowling, Bible discussion, gardening, coffee circles, Friday fest, monthly evening activities, baking group, music therapy, current events, movie of the week, indoor picnics, bingo, pet therapy, professional entertainment, one full-time activity professional, and private, secure courtyards
Fountain West Skilled Nursing and Rehabilitation Center features spacious, private, and large semi-private suites designed with you and your visitors in mind. The suites offer plenty of room for your family and friends to visit. Many patients are able to do their therapy exercises in the privacy of their own suite.
Each suite features a flat-panel TV with built in DVD player for your enjoyment. Complimentary wireless Internet, cable, and telephone service allow you to remain connected to friends and family during your stay. We know that your number one goal is to return home, and we believe that feeling at ease in your surrounding is key to maximizing the healing process. We also have unrestricted visiting hours.
Getting you home quickly is only half the job. We want to be certain that you have the strength, support, and education to remain in your home. During your stay with us, our professional staff will educate and coach you through skills such as proper body mechanics, fall prevention, safe exercises, adequate nutrition, and medication techniques. Our social services team will help you plan for your return home and even assist with obtaining home health services or other support services. We’ll help you stay home.
We provide services to many different types of clients with many different complex diagnosis. That said, there are some commonalities among the people we serve; each person wants to improve their health and ability to function independently and ultimately return home as quickly as possible.
In a typical year, we assist over 150 people in returning back to their prior home following their skilled stay. About half of our clients we admitted for therapy services such as physical therapy, occupational therapy, and speech therapy. The remaining portion utilized our advanced nursing services for treatment of wounds, pains, etc.
At Fountain West, you can expect to be treated with dignity and respect. You are our customer and we want your experience to be productive, therapeutic, and enjoyable. We want your family and friends to be a part of the team and we encourage them to visit often. We will answer your questions and give feedback on your progress.
Your room will be pleasantly furnished; television and phone service are provided at no charge to skilled clients. You can expect consistent, knowledgeable professional staff that genuinely care and have the desire to help you reach your discharge goals.
